Terms and Conditions
To receive government funded vaccines, I agree and confirm:
1. The Vaccine Coordinator and Back-up Person have completed the Cold-Chain Management - Clinical staff/Vaccine coordinators eLearning module in the last 12 months.
2. Vaccine will be stored in compliance with the National Vaccine Storage Guidelines, Strive for 5, 3rd edition.
3. This form has been completed accurately and fully. It has been at least 14 days since the last vaccine order was placed on this account and this order includes enough vaccine to comfortably provide your service plus a small buffer until the next order is received. Any orders placed within 14 days of the previous one will be rejected.
4. The TagAlert® temperature indicator with the delivery will be immediately checked. All vaccine deliveries must be accepted regardless of TagAlert® status.
5. Vaccine will immediately be unpacked into a purpose-built vaccine refrigerator and store in its original packaging. Vaccine from a breached delivery will be isolated, from other vaccine, in the purpose-built vaccine refrigerator.
6. The provider will notify Onelink within 24 hours using the TagAlert® breach report form if the arrow on the LCD screen is indicating a breach (arrow pointing toward red x).
7. No vaccine will be discarded without advice from the Department of Health.
8. A vaccine cold chain audit will be undertaken on request of the Department of Health.
9. All immunisation encounters will be promptly reported to the Australian Immunisation Register (AIR).
10. Victorian Pharmacist-Administered Vaccination Program guidelines at Pharmacist immunisers will be followed (pharmacy accounts only).
